Broward Center of Holistic Medicine(“we,” “us,” or “the Practice”) operates this website at https://www.browardcenterofholisticmedicine.com. This Privacy Policy explains what information this website collects, how we use it, and the choices available to you. Clinical care and patient health information are governed separately by our HIPAA Notice of Privacy Practices.

Zero PHI policy for this website

This marketing website is notan intake, portal, or chart system. Do not submit protected health information (PHI) — symptoms, diagnoses, medications, lab results, insurance identifiers, dates of birth, images of wounds or prescriptions, etc. — through the contact form or by email. If you need to share clinical information, schedule a visit or use the secure patient portal provided by our scheduling/charting system.

Information we collect

Information you provide

• Contact form: name, email address, and the body of your message. We use this only to respond to your inquiry.
• Newsletter: email address and, optionally, first name, used solely to send newsletter broadcasts from the Practice. Unsubscribe at any time from the footer of any message.
• Booking link-out:when you click “Book,” you are redirected to a third-party scheduling tool (Acuity, SimplePractice, or Jane) operating under its own privacy policy and, for clinical use, a Business Associate Agreement with the Practice.

Information collected automatically

• Log data: IP address, user agent, referrer, and pages visited, kept for security and abuse-prevention purposes.
• Analytics: we use privacy-respecting analytics that do not set cross-site identifiers or share data with advertising networks. We do not use Facebook Pixel or Google Ads retargeting.

How we use information

• To respond to questions you send through the contact form.
• To send the newsletter you requested, and only until you unsubscribe.
• To secure the site against abuse, fraud, and automated attacks.
• To improve site structure and content based on aggregated, non-identifying usage.

We do not sell or rent personal information. We do not use your information to serve advertising.

Service providers

The following third parties process limited data on our behalf under contracts that restrict use to our instructions:

• Resend— transactional email (contact form replies) and newsletter delivery.
• Sanity— content management; does not receive patient or form submission data.
• Vercel— website hosting and CDN.
• Scheduling provider— Acuity / SimplePractice / Jane, operating under a BAA for clinical scheduling.

Cookies

We use a small number of first-party cookies strictly necessary for site function (for example, to remember if you have accepted a notice). We do not use advertising or cross-site tracking cookies. You can clear cookies at any time in your browser preferences.

Your rights

Depending on where you live, you may have the right to request access to, correction of, or deletion of the information we hold about you. To make a request, email us at livehealthybewell@gmail.com from the address associated with the request. We will respond within thirty (30) days.

Children

This website is not directed to children under 13, and we do not knowingly collect personal information from children. Pediatric patients are seen only through the Practice’s clinical workflows, which are covered by HIPAA and handled outside this website.

Security

Submissions are transmitted over TLS. We retain contact-form messages no longer than necessary to respond, and we do not store them in the website itself. Administrative access is protected by multi-factor authentication.

Changes

We may update this Privacy Policy as the site or our providers change. The effective date at the top of this page will be updated when we do.
